HCGH Advanced Care Planning

Advanced Care Planning will help attendees think through their health care wishes, complete an advance directive and electronically store documents for health care providers to easily access. Advance directives, which can include living wills, inform providers of patients’ wishes in case they are unable to communicate. The class is free and does not require registration.
